      Ok, here is another question. At this point, would it greatly benefit him to go to Moffitt in Tampa? Since at this stage the nest step would be Yervoy. I am not seeing many other options except wait and see. Would they do anything differently you think? I just don’t want to delay the start of Yervoy waiting for an appointment over there. Any opinions?

          Moffit is probably worth it if you might ever consider doing clinical trials or you want the opinion at an institution who sees a lot of melanoma cases.  It would probably have the most to offer in that neck of the woods.  You might consider just scheduling an appointment for a second opinion – even if you feel you must start ipi now – to at least get into their system.  Chances are you'd get in faster in the future if you had already been seen.  I might consider calling and see when they could get  you in and consider waiting if it isn't too long out.  At least with the Interferon clinical trial, the data is based upon starting the trial with 53 (or 63?) days after surgery – not sure what the criteria was in the ipi clinical trial for adjuvant therapy.  Yervoy takes a long time to really get going and techinically, he is NED now so waiting a little extra time is actually feasible despite the 2 week window the doc gave you.  You do need to consider that certain treatments will disqualify you for other treatments in the future.  There's always this feeling of urgency to get started, but sometimes it is actually better to evaluate all options first – and Moffit would be a good place to do that.

                      I am so sorry to hear about your son. My dad is Stage 3c o I am wondering if they would recommend Yervoy to him since he is more advanced. Also, maybe someone could answer this, is there a risk with doing Yervoy because you would be excluded to take other drugs down the line? Could that have been why Moffitt gave the recommendation of interferon?

                      I ended up calling Moffitt and we sent in the full report. We will hear back on Monday about appointment time. Mayo Clinic said they had a 4 week to 3 month wait to get in.

But I guess that's a question for an expert. Moffitt told us that many institutions are not using interferon but they remain an advocate for it. In my opinon the research doesnt back up interferon. The melanoma specialiist at Mayo did not think interferon was a good option. He said if my son wants adjuvant treatment then he recommends yervoy based upon the recent stage 3 study. I would think it is a good option for stage 3c certainly. The study consisted of 1/3 each of 3a, 3b, and 3c. In regard to appointments and length of time to get in… my son was referred by his surgeon and he was called almost immediately to schedule the appt at Moffitt. I had help getting him into Mayo as well. I wish you the best with your father. It has been a rough ride for our family but we are hanging in there and staying strong.

                                Yervoy will not exclude you from other FDA approved drugs, it the clinical trials that have exclusions based on previous treatments or even having done ANY treatment.  That's why it's important to think ahead.  Plan A is great and if that's all you need, fantastic.  But if you have to start looking at plan b or c, that's when things can become more complicated.  Since the pd-1 drugs have been approved, there may not be as many drugs with exclusions in trials, but it always pays to think ahead and ask questions.
