3 weeks ago I had 3 suspicious moles removed and today the dermatologist called and said that the one on my back/flank area was melanoma. She said it was shallow at .2 mm and that I would need to come in next Wednesday to get more removed to check the margins. I am so anxious and trying to figure out what all of this means? I seriously feel like I might be about to have a panic attack. You can't really get a staging until they remove the tissue around is that correct? Will it take another 3 weeks to get those results??? Thank you so much for any insight.

          It depends.  frown

From the way your doctor phrased it, it sounds as if the deep margin was clear (they didn't slice through the melanoma, but under it).  If this is the case, then the 0.2mm is very, very shallow and the wide local excision will be all that's necessary, and you will be cancer-free.  No scans, no lymph node biopsies.  Now, if the melanoma extended to the deep margin, then you won't know depth until after the WLE.

Is there any way you can get your hands on the pathology report?

Thank you for your response. I can definitely call and ask for the pathology report. I will do that tomorrow! How would they know that it is .2mm if it did extend to the deep margin? What would I look for on the pathology report? She said I wouldn't need radiation or chemotherapy, how would she know that?

In general, even shave biopsies go deeper than .2mm.  It's hard to remove that little amount of skin.  So the chances are your deep margins are clear.  Most likely stage 1a.  If the deep margin is clear, then staging is basically based on the biopsy itself, not after they remove additional tissue.  It's rare to find anything on the wide excision that changes the biopsy results on a lesion like this.  Stage 1 has a fantastic prognosis.  Remove more tissue to achieve WIDE margins (guards against a stray cell being missed) and then you move on.   As long as the pathology report says your deep margin was clear, you really don't have to worry about the pathology report on the wide excision — even if it takes 3 weeks.  It isn't going to change anything.  It's a shock, but it looks like you caught it very early and are in a good place.

Thank you so much! I think it was a punch biopsy. Your words have made me feel a little better 🙂
