my mom wanted to get lag3 and pdl1 clinical trial treatment but they wanted to get the biopsy to do molecular testing .today we got this result from her pathology report :Given the extensive necrosis, this specimen would not be appropriate for molecular testing. The tumor that they did the biopsy shows extensive deal tissue . She was on keytruda but her tumor progress never shrinked. But now it shows dead melanoma cell. Does this mean keytruda is doing something ?

Hi Lilara,I think you need an expert to respond, but no one responded so I will try. Necrosis is obviously (I think) a good thing, so whatever medicine she was taking has had an effect. I cant remember all the details of your mum’s case but I think you were worried treatment was not working and you were being denied ipi/nivo or something else.
With my tumour the oncologist often hoped it was necrotic but every time we did a biopsy it came back as melanoma.
My only question is, although this is a good sign, maybe they have to re-sample to find a part of the tumour that is not necrotic. So although necrosis is good news, I am not sure that it means the whole thing is necrotic. And are there other tumours? So they may have to re-sample. (But these are all guesses, sorry. At least you have some good news!!!!)

Thanks Mark for your response . My mom was on keytruda but the tumors were not shrinking so the dr is looking at her tumor to see if she is eligible for pd1 and lag3 , since she was denied ipi/nivobut now at her biopsy they have found extensive necrosis . So we will see the dr on Thursday. So I hope the cancer cells are dying . But I don’t know why her tumor is growing :-(. Thanks for your response . I am praying all the time so that she can have something that works for her .

Hi Lila,How many tumours does your mum have? If she has more then maybe they will do the biopsy on another one, and then they can do the pd1/lag3. (Why denied ipi/nivo?).
But if she only has one and has necrosis then you might have good news!!! It can grow because the tumour becomes inflamed (I think). Keytruda worked for me on a small tumour but only after that thing grew a little which freaked me out, but actually this was good news it then shrank. And I think on my other one, with TVEC it grew a little before slowly shrinking.
Dont be overoptimistic – it may just be they take a new sample (I am not an expert!!!!). But maybe you have good news. I dont see how necrosis can be bad news!
