Hello,
I live in Toronto, Canada and I have stage 4 melanoma. I have a single tumor on my pancreas that measures 8cm in length. I have been dealing with it since 2012. So far it has not spread beyond the pancreas but it continues to grow and will soon cause problems. I did Interferon for 7 months in 2010 when I was first diagnosed.
The options for me in Canada are;
1. BRAF – I am BRAF positive (Vemurafinib or Debrafinib)
2. Ipilimumab
3. Clinica Trial – includes a BRAF and a MEK Inhibitor
If I fail on a BRAF and Ipilimumab then I qualify for Anti-PD1 under compassionate use. It is not yet approved in Canada.
The other option would be to look for a clinical trial with Anti-PD1 in the US. I am looking for some informed opinions on how I should proceed. Also, if anyone knows of recruiting clinical trials in the US I would be interested.

You might want to consider a ipi nivo trial. I believe they have them running at mofffitt in Tampa. The data suggests good results but side affects can be serious. You need a really good team to monitor things. They also got an arm where they reversed where the pd1 is higher dose and ipi smaller so might be less side affects.

I am Braf negative so I'm not in the same boat. However I have tried surgery, radiation, Yervoy, a clinical trial at NIH in Bethesda, and now I'm getting Keytruda infusions. I've had the best luck with Keytruda. Since you live in Toronto, a trip for an opinion at Memorial Sloan Kettering in NYC might be an idea.

Hi Mark, there is one more thing that you can look into at the Princess Margaret in Toronto. They are now a location for TIL (Tumor-infiltrating lymphocytes) therapy. Dr.Pamela Ohashi is in charge of program. Two Oncologists involved are Dr. Anthony Joshua and Dr. Marcus Butler.You can look up information about TIL treatment, by looking for a video on youtube called "Adoptive Therapy for Melanoma " Melanoma Education Symposium by Patrick Hwu MD. from MD Anderson in Houston, Texas. Dr. Hwu is one of the leading figures in this type of treatment. Wishing you the best. Ed

all great recommendations above and another doctor offering hope & treatments; Dr. Hamid Los Angeles Clinic 310-231-2135, 310-294-0438.
he made it all work for me – my physical , emotional & economical health
my insurance would not participate in clinical trials – he retained Merck to cover all aspects of treatment- metrics, labs, clinics, antiPd1 etc….
I covered travel from Tacoma, Wa / there was even some reimbursement of lodging
