Two abstracts addressing ipi 3mg/kg and 10 mg/kg in both Stage IV melanoma and Stage III now up on the blog. Spoiler alert: No difference in response….big difference in side effects at 10!! Cool link to Dr. Weber addressing the same (as well as 4 other reports) in a video discussion from the Edster on the post and also on the "ASCO brain met" link below. Are you listening FDA????? Have a great Monday, everybody. C

I had 5 doese of ipi at the 3mg/kg dose. I had lots of side effects but none to the point of stopping treatment. I had a great response.

Hey H…
I see from your post above that you took the ipi/nivo combo…which is a little different from the folks treated in the abstracts I posted as they took only ipi or ipi with interferon…with ipi at either a dose of 3mg/kg or 10mg/kg. None of these peeps got nivo. Just didn't want to confuse anyone. Nevertheless, so glad of your good results and hope that the kidney cyst is nothing more than that and your side effects are mangaged as best they can be. I wish you well. c

Like everybody else I just want to say thank you. The time you put into it must be exhausting not just mentally but physically. You make it so much easier for all of us. My question is in your research did you notice when they actually started your response time. My husband hado surgery late September 2014, and started IPI in Nov 2014 and finished 2015. We are truly bless3d to say he has been stable since. Scans due in Aug. Thanks for any and all replies.Jewel

Hi Jewel,
I am glad to hear of your husband's continued stability and send good wishes for his upcoming scans. I am not entirely certain what you are asking. The two abstracts in this post do not address time to response, they deal primarily with progression and relapse free survival. For most folks in an adjuvant study, they are NED at the start…so knowing when a "response" kicks in is almost impossible to determine….and one can only measure how long they remain disease free. For ipi historically, like anti-PD-1 (immunotherapy in general) time to response, for folks with measureable disease, is generally at about 12 weeks. However, there are those who can gain a response much later. Here is a graph that tells the tale: http://chaoticallypreciselifeloveandmelanoma.blogspot.com/2016/02/time-to-responseipi-vs-nivo-and-ipi.html
As you can see…the ipi only folks are at the bottom. Most of the response dots fall at about 12 weeks…but there is one person out at past 32 weeks.
Hope this helps. C

Hopefully I will be able to attest that 3mg of ipi works for Stage3! When initially diagnosed with Stage 3a, I saw Dr. Kirkwood and his only recommendation was interferon. My local oncologist recommeded a 2nd opinion, he knew interfon was an old drug and he said he never had anyone finish the interferon treatment that he gave it to. I got a 2nd opinion from Dr. Schucter from Philadelphia and she recommeded no treatment but if I didn't want to go that route, we could see if my insurance would approve ipi at 3 mg and my insurance did. I have tolerated it very well, just minior intestinal rummbling a few days after my treatment and that is it. I am going to receive my 3rd maintenance dosage at the end of this month and then scans in August and last maintenace treatment in September / October. I am currently 1 year NED.
