      my husband (37/m Caucasian) had a mole removed from his back and just got the results:

      Diagnosis: fibrous papule

      Compound melonocytic Nevus with architectural disorder and severe cytologic atypism of the melanocytes (displastic nevus, severe)

       

      Compound melanocytic Nevus with elongated, fused rete edges, extensions of the junctional component beyond the dermal portion and concentric lamellar fibrosis within the underlying papillary dermis. A mild superficial lymphocitic infiltrate is identified. Cytologic atypia is severe. 

       

      we are very worried. Doctors are going to remove more of the area. It’s close to his spine. Wondering how worried we should be? If we should get second opinion and do further testing.

          This is not considered melanoma, this is a severely dysplastic / atypical nevus.  It is very atypical, however, and had a higher likelihood to become melanoma than other atypical lesions.  The standard of care is to remove with 5mm margins.  These are the same margins they use for melanoma in situ.  That is the first stage of melanoma (stage 0).  Since it can be a bit of a judgement call by pathologists, they err on the side of caution and have 5mm margins for both.  So, it's not all bad news at all.  You don't have a cancer diagnosis but you treat aggressively anyway – just in case.

          You can get a second opinion, but as long as your doc removes with margins, I'm not sure you really gain anything.  After it's removed, you just watch for any pigment regrowth in the scar.  Watch of other new or changing moles that are the ugly ducklings.  And move on – you caught this before it became anything worse!

              Thank you kindly! Greatly appreciated. He'll be getting regular biannual screenings afer this. I lost a close family friend to melanoma last year so was quite scared by this.
