I've been on taf/Mek for 17 months with amazing results until this last month I had 4 new brain mets. It seems to be working in my body still but not my brain. I also just started ipi/nivo combo and my doctor wants me to continue taking the BRAF meds for at least another month. I'm a little nervous about toxicity with all of this in my system. Does anyone have any experience with these taken together? Or know of any articles or studies combining all 4? Thanks for the help!

Sara – First of all – sorry to hear about the mets – and thank goodness modern medicine seems to be providing some options! … when I was diagnosed as IIIC this Fall and told about the various targeted therapy & immunotherapy options – I did ask about combinations. My doctor said the toxicity of 4 drugs at the same time has not been tested and is probably too high/dangerous.
Have you experienced strong side effects from the Taf/Mek? Do you know if your doctor has managed other people on two treatments/4 drugs concurrently? … these are questions I would ask.
(I also think there is an insurance component, not sure if insurance would approve 2 concurrent treatments – but that is opening an entirely different conversation and if you still have the Taf/Mek on-hand, realize this isn't an issue per se).
Wishing you the best in the next steps.

Speaking from experience, not articles or studies. Ipi with MEK is not good combo for toxicity. BRAF/MEK with PD-1 is fine. I'd probably skip the BRAF/MEK for the ipi portion of the treatment and some time thereafter. I would get the brain mets g-knifed prior to starting ipi/nivo.
