A. Right mid paraspinal: Malignant Melanoma without ulceration, clark level 3, 0.45 mm in breslow thickness, with a mitotic index of 0 (pTla).
Tumor Anatomic site: Mid paraspinal.
Histologic type: Superficial spreading melanoma.
Maximum tumor thickness: (Breslow) 0.45mm.
Anatomic level: Clark Level 3
Ulceration: Not identified.
Margins:
Peripheral margins uninvolved by invasive melanoma. Distance of invasive melanoma from closest peripheral mergin: 1.4mm. Peripheral margins uninvolved by melanoma in situ. Distance of melanoma in situ from closest peripheral margin: 0.9mm. Deep Margin uninvolved by invasive melanoma. Distance of invasive melanoma from deep margin: 0.5mm.
Mitotic Rate: not identified
Microsatellitosis: Not identified
Lymph-vascular invasion: Not identified
Tumor infiltrating lymphocytes: Not identified
Tumor regression: Not identified
Growth phase: Radial and focally vertical.
Additional patholog findings: Dermal nevus.

Great results. Sucks to ever have to hear the word melanoma, but this report shows that it's a stage 1a, not very thick and no ulceration (which is good). Now, your brother will go for regular dermatologist skin checks every year (probably every 6 months for a while) and he will be aware of any more new things that pop up in the furture and get them biopsied. It's really the best report one can get besides in situ (stage 0).

Now, your brother will go for regular dermatologist skin checks every year (probably every 6 months for a while) and he will be aware of any more new things that pop up in the furture and get them biopsied. It's really the best report one can get besides in situ (stage 0).

Thank you so much for the reassuring news!! It truly means a lot! When I was on the phone with my brother he thought the doctor said 4.5mm breslow level the first time he talked to him so I was freaking out!

Understandably… that's why it's always best to wait for the report.. since our brains don't always retain info well after hearing words like melanoma.

You as well!!! Also one more question if he has had one melanoma does that mean that he is more susceptible to get them in the future? Or not necessarily??

Yes, but it is still unusual for someone to have more than 1 primary melanoma. Seems like he is already aware of his body and new spots, so he'll know to get anything new checked out. Even if he had multiple stage 1 melanomas, that's lightyears better than having 1 mel that spread ๐

Went back to doctor today to have a closer look at his body. He wanted 2 more off and the doctor wanted 4. So he got a total of 6 taken off! I'm hoping none come back worse than this previous case and that they are either stage 1 or just altogether normal.

Most likely they will come back as nothing to worry about, probably atypical. Hopefully he will be done getting cut up for a while after this, poor guy.

when you say it is unusual for him to have more than 1 primary melanoma what does that mean? How does one know if they have other melanomas or which is the primary?? Thanks so much just kinda lost!

Primary is the original tumor. So, for melanoma being a skin cancer, the original tumor shows as a mole or skin lesion. So, if he has another mole on another part of his body that gets biopsied and is melanoma, then it would be a separate occurance, not related to the first, and would be considered a second primary tumor. No way to know if another mole we have is melanoma, other than looking for signs like change and the "ugly duckling" scenario, which is a mole that looks different from all other moles on one's body. Things like that are cause for biopsy to investigate if it's worriesome. My dermatologist, who is a melanoma specialist, told me that if I were to have another primary melanoma, meaning a new mole show up on my skin that is melanoma, it will look very similar to my first one. Usually people will not have a second primary be a different type of melanoma or look much different from the first.
