Melanoma staging is the method used in describing the extent of the disease. The doctor will look for the tumor thickness and depth of melanoma cells to the different parts of the body. It helps in determining the treatment plan and the prognosis (outlook). Stage IV, defines that the cells of cancer have been spread to the lymph nodes or to other organs in the body which generally represent metastatic melanoma. Either they are removed surgically or have treatments like targeted therapy, or immunotherapy.

Well though atypical moles are considered as pre-cancerous, but every atypical moles doesn’t get melanoma. Even they never become cancerous; however, they can be a warning of developing melanoma. The initial diagnosis of atypical moles is a physical examination, however, sometimes a procedure, called biopsy, performed in pathology where the tissues are examined under a microscope. It will help in getting substantial cure for the risk of developing melanomas.

I think you really need an advice of a good doctor asap. Melanoma is considered as a cancer, which develops in melanocytes i.e. the pigment cells occur in the skin. It is estimated that every year more than 50,000 new cases of melanoma diagnoses occur in the United States. The only way to confirm whether your suspicious looking mole or spot is a melanoma or not is being tested. Doctor will generally recommend diagnostic excision (a type of skin biopsy) in which the excision skin sample are removed to examined under a microscope. It will be helpful in confirming whether you have a disease or not.
